RMA Form
1: Click on the RMA form link below to download and print the blank form. You can send an email to: rma@simmtec.com or call our RMA Dept. for assistance. Hours are: Mon - Fri 10 am - 3 pm Pacific Daylight Time.
2:Fill out the RMA request form completely and correctly with the details of your information, item code, quantity, serial #, invoice #, invoice date and the brief description of the defect.
3:Fax the completed RMA request form, copy of the invoices, and the serial number sheets (if needed) to 510-249-1430.
4:Affix the packing slip of the 1st RMA (as a proof of purchase) if requesting for 2nd RMA.
5:Wait at least 24 working hours after you fax the correct documents for either a RMA number or a reason to refuse.
6:Contact our RMA Dept. for assistance if you do not receive any response to your request within 24 working hours.
7:Do not fax duplicate request. All information will be disregarded without notice.
8:Ship the copy of RMA approved form with the defective parts (no accessory needed, except all Retail Box CPUs). Attach the fan and box w/ the defective Retail Box CPUs
9:Mark the approved RMA number clearly (if there are more than one number, please mark all) on the shipping label on the outside of the package.
10: For Will-Call customers: No pick-up or delivery of RMA's will be accepted after 3 pm daily.
